Except as provided in this statement, InstaMed does not use personal information for a purpose other than the stated purpose for which such information is collected.

• Please have your statement on hand when you call. Due to the Heath Insurance Portability & Accountability Act (HIPAA), you will be asked to verify your date of birth and social security number.
• Please remember that Shady Grove Radiology provides the professional component (reading) of your radiology exam performed at Shady Grove Adventist Hospital. Our bill is not included in the facility charge.
